Attendee Registration and Information

There are two ways to register for the 2008 conference: online or by mail. Credit card payment must be used when registering online. Visa, MasterCard, and American Express are accepted. Registrations submitted online are not considered complete until payment is processed. Please note: AADMD members must register by mail or fax.

Non-members may use either form. Fill in the appropriate information and mail with a check made out to DDNA, to the following address: DDNA 2008, P.O. Box 536489, Orlando, FL 32853-6489. If you are unable to print pdfs, please contact DDNA at 1-800-888-6733, and we will fax or mail you a registration form.

Group Registrations

If you are registering a number of people for the conference online with a company credit card, you must register each person individually. If they are DDNA members, you must be logged into the DDNA website using the member's username and password for the member rate to be available. Alternately, you may call DDNA at 1-800-888-6733 with a credit card number and the names of attendees.

Conference Registration Fees

Registration includes admission to all sessions. Included in the reigstration fee are the reception, continental breakfasts, luncheons, and refreshment breaks. Pre-conference registration is additional.
Cancellation Policy: Registrants cancelling by May 23, 2008 will receive a full refund minus a $45 administrative fee. Cancellation by May 30, 2008 - 50% of fees paid will be refunded. No refunds after May 30, 2008.

Making Hotel Reservations

The MGM Grand Hotel is offering DDNA and AADMD a substantially discounted group room rate, which is available only to registered attendees and their guests during the 2008 Educational Conference. The rate is $155/night plus tax, and rooms are available from June 4th through June 10th. Cut-off date for the discounted registration rate is May 14th. We recommend that you register for your hotel room as soon as possible after you register for the conference, because the Foxwoods Resort is a popular destination. To receive the discounted group rate, you must first register for the conference, then make your hotel reservations at the MGM Grand Hotel. The discounted rate is available only for the MGM Grand Hotel, not for any other Foxwoods Resort hotel.
